If your company was overcharged for Ohio Workers’ Compensation premiums between 2001 and 2008 resulting from disqualification from the group rating program, you may be eligible for a refund of the overcharges IF AND ONLY IF you properly file a claim by October 22, 2014. If you received a claim form from the BWC you should fill it out and return it to the Claims Administrator, postmarked no later than October 22, 2014. A COMPANY MAY MAKE A CLAIM EVEN IF IT IS NO LONGER IN BUSINESS so long as the required information is provided by an authorized company representative. Payments are scheduled to begin in February, 2015.
If you do not have the form it can ONLY be obtained by calling the Claims Administrator at one of the numbers listed below and identifying the company name, address, BWC number and EIN number. Some potential claimants have experienced problems obtaining the claim forms from the Claims Administrator.
